INSTRUCTIONS TO CANDIDATES FOR COMPLETING THE GRADUATION PROCESS
 

Instructions to College of Education Candidates for Completing the Graduation Application Process

 

The following check list is provided for your information and use as you progress toward completion of your undergraduate degree requirements.  The factors indicated below are those which frequently cause delay or disapproval at the time of the final graduation check.  Please review this list and check off the items as they are completed to insure that your graduation may be accomplished on schedule.  Should you have questions, please contact Melanie Shaffer at 859/622-1828 or by email to Melanie.Shaffer@eku.edu.

 

Graduation Fee

 

The graduation fee of $20 will automatically be assessed by Billings and Collections.  This fee covers the cost of your cap and gown, diploma cover, and alumni fees for one year.

 

Transcript

 

An unofficial transcript will be requested for you.

 

CARES report

 

Obtain a current copy of your CARES Report from the Banner Web. 

Questions concerning your CARES Report should be directed to Melanie Shaffer, Tracking Specialist .

 

§  Is your major / minor identified correctly on your CARES report?

§  Does your CARES Report accurately reflect your progress toward completing program requirements?

 

On-line Application for Graduation

 

Complete the College of Education on-line Application for Graduation and print a copy before you submit.   Note:  The application process is not complete until you meet with the Tracking Specialist.

 

Bring a printed copy of your completed application to your graduation exit review appointment. 

Failure to complete the form carefully and completely may delay your graduation.

 

§    Name will appear on your diploma as indicated on your  application.

§    Diploma will be mailed to your permanent address as indicated on your application.

§    Indicate the exact graduation date for which you wish to be considered.  If you do not meet program and degree requirements for the graduation date indicated, we cannot move your Application for Graduation to the next graduation list until you notify us, in writing, to do so.

§    Check the accuracy of the degree program identified on the application.

 

Enrollment

 

Complete the enrollment form and print a copy.  Bring a printed copy of your enrollment form to your graduation exit review appointment.

 

Alumni Association Activities and Organizations(P1)

 

Print and complete the EKU Alumni Association/EKU Activities and Organizations form   Page 2 Also must be printed. Bring the completed activities/organizations form to your graduation exit review appointment.

 

Required Surveys

 

Your satisfaction with, and reactions to, your education and student life experiences at Eastern are important to us, and will be used in educational planning.  Because each survey is anonymous, you must complete each survey in one sitting.  You cannot save a partially completed survey for completion at a later time.  The completion verification form will be generated by the online survey software after you have completed and submitted your responses to the survey.  You must print out the completion verification form immediately after completing each survey.

Lost completion verification forms cannot be replaced.  You will receive one and only one completion verification form.  In order to protect the anonymity of your responses to the survey, duplicates of the form are not kept or generated.

 

Exit Examinations

 

Have you registered for or completed required PRAXIS examinations and the *PLT exam for teacher certification? For additional information regarding PRAXIS, visit Praxis Information Remember.....PRAXIS is a graduation requirement! You must take the PRAXIS to graduate.....Pass the PRAXIS Exams to be certified. *IECE majors are not required to take the PLT exam.   Questions concerning PRAXIS exams or PLT exam should be directed to Melanie Shaffer 859.622.1828

 

*IECE majors do not take the PLT exam

 

Student Teaching Application

 

Have you completed the application process for student teaching?  Questions concerning the application for student teaching process should be directed to Mrs. Rose Skepple, Assistant Director of Professional Laboratory Experiences (859/622-1544  - Bert Combs 425).

 

Required

Graduation Exit Review Appointment

 

You must schedule an appointment with Melanie Shaffer, Tracking Specialist to review that all program and degree requirements will be satisfied by the expected time of graduation.  To schedule an appointment you may call 859/622-1828 or stop by the office in Bert Combs 423.

 

To be considered a candidate for graduation, return the following at your scheduled appointment time:  NOTE: Your application will not be considered unless all components are returned.

  • Submitted an on-line Application for Graduation and printed a copy

  • Enrollment form

  • Un-official Transcript will be provided                                                       

  • CARES Report

  • Alumni Association form

  • Completion Verification form  for University Survey of Undergraduate Degree Candidates

  • Completion Verification form  for College of Education Survey of Undergraduate Candidates

  • Completion Verification form  for Teacher Education Student Technology Questionnaire
